Nearly every bubbled, peeling floor I get called to look at has the same cause. Water vapor moving up through the slab, pushing on a coating that was never given anything to stop it. It is invisible, it is testable, and it is fixable before you spend money on a floor.

Concrete sits on dirt, and dirt holds water. That water evaporates up through the slab as vapor, every day, whether the floor feels dry to the touch or not. On an uncoated floor it passes through and you never notice. Put a coating over it and that vapor has nowhere to go, so it collects underneath and pushes until the bond gives up.
That is why a coating can be installed perfectly and still fail. The prep was right, the product was right, and the slab underneath was working against it the entire time. The only way to know is to test.
This is not a product I upsell to everybody. It is a test result that either says you need it or says you do not:
The obvious one. Below-grade slabs sit surrounded by damp soil on every side and this is where vapor drive does the most damage.
Poured directly on soil, often with no vapor retarder underneath if the house is older. Very common in older Kansas City housing stock.
If a coating already lifted off this floor once, moisture is the first suspect and testing tells you before you repeat the mistake.
Large slabs, high stakes. A failure here means production downtime, not just an ugly floor.
Plenty of contractors sell a moisture barrier on every job because it is an easy upcharge. I test first and tell you the result, including when the answer is that your slab does not need one. Selling you a barrier you do not need is just a more expensive way of being dishonest.
What the barrier does and does not do is also worth being clear about, because it gets oversold in both directions.
A vapor barrier stops moisture moving through the slab. It does not stop a leak, fix a bad grade, or dry out a wet basement. Those are different problems with different fixes.
These systems are rated to a specific pressure. A slab pushing harder than the system is rated for needs a different conversation, and I will have it with you rather than install something I expect to fail.
Slab moisture moves with the season and the weather. A test in a dry August is not the same as one in a wet April, and I will factor that into what I recommend.
There is no shortcut that skips grinding. If someone offers to roll a barrier over your existing floor, that is not what this is.

The simplest is a plastic sheet test, taping a sealed square of poly to the slab and reading what collects underneath. For jobs where the stakes justify it, calcium chloride or in-situ relative humidity probes give an actual number. I use what the job calls for and I tell you the result either way.
No, and I will tell you when yours does not. Plenty of slabs test dry and the coating goes straight down. The barrier is a response to a test result, not something I add to every quote.
No. A vapor barrier stops moisture moving up through the concrete. It does not stop water coming in through a wall, a crack, a bad grade or a failed drain. If you have an active leak, that is a different trade and it should be fixed first.
It is the most likely one. When a coating lifts in sheets or bubbles across the middle of a floor rather than at the edges, moisture drive from underneath is the usual cause. Testing the slab before recoating tells you whether it will happen again.
Not without removing what is there. The barrier bonds to bare concrete, so the existing system has to come off by grinding first. It is real work and it belongs in the quote honestly.
